Before Endodontic Treatment

If this is your first visit to our office, you will need to fill out some paperwork before your treatment. You may fill out your health history and HIPPA form online using our secure server. Just go to the "online forms" section of the website to get started.

Before your treatment…

• Please eat a regular meal
  
• Please take your medications as regularly prescribed
  
• If are required to take antibiotic premedication before your dental treatment, please premedicate one hour before your appointment. Please be aware that the recommendations have recently changed regarding antibiotic premedication. For more information regarding these new recommendations from the American Heart Association, please click here
  
• Please feel free to contact our office with any questions you may have about your insurance and/or financial arrangements
  
• If your referring dentist has sent a referral slip and/or x-ray, please bring it to your appointment
