Your First Visit
At your initial appointment our amazing doctors and friendly team will review your medical & dental history, perform a radiographic x-ray and clinical examination.
Consultations and treatment can be done all in the same day on certain procedures. Our professional front office team will help you with all your scheduling needs.
Please assist us by providing the following information at the time of your initial visit:
- Your referral slip and any x-rays if applicable
- A list of medications you are presently taking
- If you have dental insurance, bring your insurance card
Please alert us if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, heart murmur, artificial heart valves and joints, rheumatic fever, etc.). Also, please let us know if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) if you have allergies to any medications (i.e. sulfa, penicillin, aspirin, etc.) or have a latex allergy prior to treatment.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Payment for care is due at the time of service unless other arrangements have been made in advance. For your convenience we accept Visa, MasterCard, American Express, Discover and CareCredit. We deliver the finest care at the most reasonable cost to our patients. We will provide you with an estimate of your insurance coverage and your estimated co-pay prior to treatment. If you need special financing, CareCredit is available in our office. CareCredit allows you to choose from a variety of payment plans that permit you to pay for your treatment over time, while receiving the treatment that you need today. We are committed to helping our patients maximize their benefits and we will work with you to acheive the maximum benefits for your coverage. If you have dental insurance we will complete and submit a claim form as a courtesy to you. We also ask patients to be responsible for the estimated portion their insurance does not cover at the time of service. The patient is personally responsible for all fees for services provided, not the insurance company. We will wait 90 days for the insurance company to pay their portion. In the event the insurance company fails to pay, rejects the claim, or does not pay the total balance, we ask that the patient make arrangements for payment. We are happy to discuss any finance questions or problems and find possible options.
Insurance we accept:
Aetna, Ameritas, BC/BS Arizona (non-Federal), Cigna (select plans), Connection Dental (GEHA, Guardian, Lincoln Financial, United Healthcare, others), Delta Dental, Humana, Met Life, Principal, Sun Life Financial, TDA, United Concordia (select plans), Veterans Administration. We accept all indemnity plans and most PPO plans. Please check with us for verification. AHCCCS, HMO and Medicare plans do not pay our office.
